A study on deriving a conversion formulae using mapping between HINT-8 and EQ-5D instruments

Abstract
The objective of this study was to derive conversion formulae among HINT-8 and EQ-5D instruments. Total 220 people were surveyed using EQ-5D-3L, EQ-5D-5L, EQ-VAS, and HINT-8 instruments and pairwise conversion formulae were derived using regressions.
The average of health utility scores in EQ-5D-3L, EQ-5D-5L, EQ-VAS, and HINT-8 was 0.94, 0.91, 0.84, 0.84, respectively. The ceiling effect was 51.4% in EQ-5D-3L, 47.3% in EQ-5D-5L, and 1.8% in HINT-8. The correlation between EQ-5D-3L and EQ-5D-5L utility scores was the highest (r=0.82), and also the ones between EQ-5D-3L and HINT-8 and between EQ-5D-5L and HINT-8 showed high correlations. The association of domains between EQ-5D instruments and HINT-8 showed pain and depression domains have high correlation coefficients, r=0.69 and r=0.78, respectively. The derived conversion formulae showed explanatory power of 57~69% and they seems to be reasonable. When converting the HINT-8 to EQ-5D-3L, the derived range was from 0.474 to 1, while the conversion range for EQ-5D-5L was from 0.259 to 1.
The derived conversion formulae from this study considered all the available health related utility instruments in Korea. The others do not have a valuation study in Korea yet. Hence, this study has an importance that comparisons among the available health related utility measures in Korea. More specifically, the comparison between the future estimates of HINT-8 in the Korean National Health and Nutrition Survey (KNHANES) and the past estimates of EQ-5D-3L in the KNHANES will be possible. Also the comparison between HINT-8 and EQ-5D-5L will be useful in patient reported outcomes studies and economic evaluation studies.
